Patrol Permission Slip

Because Patrol is run by a student group not affiliated with ESP, you will need a permission slip signed by your parent/guardian in addition to the medical/liability forms you will bring with you to Splash registration. DO NOT hand this in during registration. Keep it with you until Saturday evening. I will be collecting them before leading you over to the Patrol area. If you don't have yours, you will not play.

Print out the following and have your parent/guardian sign it:

I hereby give _ _ _ _ _ _ _ _ permission to play in the MIT Assassins' Guild game of Patrol, which runs every Saturday evening on the MIT campus. I understand that it is a highly active game equivalent to a team sport, and that injury is possible as it is for any sport.
