The form at the link below has the primary instructions for how to submit a claim that there is a Lost Item for which you would like compensation. Here are, however, some key points:
- If you are under 18, please have a parent or guardian submit the form.
- Itemize (that is, list separately) a description and value for each item you bought or brought but did not get back. This includes any payment for the use of extra dye for
- If you have already received compensation without this form, please submit a claim form with an "item" showing the amount you have been given. Please submit this, even if the total amount you need compensated is zero. This is for our records.

Dear student:
Our records show that you attended our "Tie-Dye ANYTHING! (all day Saturday)" class at our recent program, Splash! 2007. A substantial number of students bought or brought t-shirts and other items to be dyed but did not get them back: these items were lost or stolen. While the loss is at best an accident, we wish to offer compensation for lost items. If you are one of the affected students, please have your parent or guardian fill out and submit the form at the link below by January 16, 2008.

We will make an effort to handle each claim within two weeks of receiving it, but please allow until the end of January for processing.
